Prepare for your visit at CSRI — what to bring, downloadable forms, insurance information, and recovery education resources.
For clinic appointments, arrive 15 minutes early with photo ID, insurance card, medication list, and athletic clothing if a movement assessment is planned. For surgical visits, arrive at your confirmed time (typically 90 minutes before the procedure) with a designated driver.
